Insurance Coverage Guide

Before your first appointment, contact your insurance provider using the Customer Service number on your card. Use this guide to determine your coverage.

Step 1: Locate Member Services Number

Find the customer service number on the back of your insurance card.

Step 2: Call Your Insurance Company

Ask the following questions:

Provider Network Status

  • Is Nutritional Glo (NPI: 1437876430) in-network for my plan?
  • If not, what are my out-of-network benefits?

Covered Services

Does my plan cover these outpatient nutrition therapy codes?

  • 97802 (assessment)
  • 97803 (follow-up)
  • 97804 (group)
  • S9470 (“nutritional counseling”, most common for BCBS®)

Covered Conditions

Which conditions are covered? Ask about these ICD-10 codes related to your condition:

  • Z71.3 (healthy eating)
  • R73.09 (pre-diabetes)
  • E11.6 (type 2 diabetes)
  • E28.2 (PCOS)
  • E66.3 (overweight)
  • E66.9 (obesity)

Your Costs

  • Is there a deductible for Nutritional Counseling Services?
    • If yes, how much? How much has been met?
  • What’s my cost-share?
    • Co-payment per visit or co-insurance?
    • If co-insurance, what’s the coverage percentage?
  • Note: Preventive care often has no out-of-pocket cost.

Visit Limits

  • How many sessions are covered annually?
  • When does the policy year renew?

Referral Requirements

  • Is a physician referral needed?
    • If yes, how much? How much has been met?
  • If yes, ask your primary care doctor to fax a referral to (888) 216 – 0062.
  • Are nutrition counseling visits covered under preventative care? (ICD-10 code Z71.3)
  • Is telehealth nutrition counseling covered?

Additional Information

  • Are there other restrictions or limitations?
  • Note the call reference number.

Network Status

  • In-Network: We will bill your insurance directly and bill you for any remaining balance.
    • If yes, how much? How much has been met?
  • Out-of-Network: Pay upfront; potential refund after insurance processes the claim.
